Release & Assumption of Risk
This is a legally binding agreement. This agreement protects Ice Wave Figure Skating School, its programs, coaches and/or any other staff or person related to Ice Wave Figure Skating School.
I hereby certify, I am aware that ice skating involves certain inherent risks, dangers and hazards which can result in serious personal injury. I understand that skating is dangerous activity. I understand the nature of the Activity, and I represent that the participant is qualified, in good health, and in proper physical condition to participate in this Activity. I understand, this activity involves risks of injury, which may be caused by my own actions, or inactions, those of others participating in the Activity, there may be other risks either not known to me at this time; and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in this Activity and/or any related activities.
I Accept
I hereby release, discharge Ice Wave Figure Skating School, its programs, coaches and/or any other staff or person related to Ice Wave Figure Skating School from all liability or actions, claims, demands, losses, or damages arising from and/or relating to my activities with Ice Wave Figure Skating School and waive the right to bring a court action to recover compensation or to obtain any other remedy for any injury to myself or my property whether I am supervised or unsupervised, now or in the future.
